Overview & Mechanism of Action
- Osmolax Solution ($200\text{ml}$ bottle containing Lactulose $66.7\%\text{ w/v}$) delivers a non-absorbable disaccharide formulated for osmotic bowel management and hepatic metabolic detoxication:
- Colonic Osmotic Hydration & Peristalsis: Lactulose passes unabsorbed through the upper gastrointestinal tract to the colon, where colonic bacteria ferment it into low-molecular-weight organic acids (lactic and acetic acids). This lowers colonic $\text{pH}$ and increases intraluminal osmotic pressure, drawing water into the lumen to soften stools and stimulate physiological peristalsis.
- Ammonia Trapping in Hepatic Encephalopathy: Lowering colonic $\text{pH}$ converts free toxic ammonia ($\text{NH}_3$) into non-absorbable ammonium ions ($\text{NH}_4^+$). This traps ammonia within the stool and prevents systemic intestinal absorption, mitigating neurotoxic symptoms in hepatic impairment.
- Prebiotic Microflora Support: Serves as a prebiotic substrate, selectively stimulating the proliferation of beneficial gut microorganisms (such as Bifidobacteria and Lactobacilli) while suppressing acid-sensitive pathogenic bacteria.
Clinical Indications & Uses
- Osmolax Solution 200ml is indicated for:
- Symptomatic relief of chronic functional constipation and bowel irregularity
- Stool softening to facilitate comfortable defecation following anorectal surgery or in conditions such as hemorrhoids and anal fissures
- Prevention and management of portal-systemic encephalopathy (PSE) and hepatic pre-coma/coma
Usage Guidance & Safety Precautions
Administration:
- Measure doses using a calibrated measuring cup or spoon. May be taken undiluted or mixed into water, milk, or fruit juice.
Recommended Dosage:
- Constipation (Adults): Initial dose of $15\text{ml}$ to $45\text{ml}$ daily as a single or divided dose, adjusted to a maintenance dose of $15\text{ml}$ to $30\text{ml}$ daily once regular bowel movement is established.
- Constipation (Children): Dose strictly according to age and clinical direction.
- Hepatic Encephalopathy: $30\text{ml}$ to $45\text{ml}$ taken 3 to 4 times daily, adjusted to produce 2 to 3 soft stools per day.
Precautions & Warnings:
- For oral administration. Contraindicated in galactosaemia, severe inflammatory bowel disease, intestinal obstruction, or gastrointestinal perforation. Use with caution in diabetic patients due to small quantities of free galactose and lactose inherent to lactulose solutions. Transient flatulence, meteorism, or abdominal cramps may occur during initial therapy. Excessive dosing leading to persistent diarrhea can cause dehydration and electrolyte imbalance. Store below $25^\circ\text{C}$ in a cool place away from direct light. Do not freeze. Keep out of reach of children.
